Fingerprinting and Background Screening Information: Information necessary to process fingerprinting or background screening services, which may include name, date of birth, place of birth, Social Security number when required, citizenship information, demographic information, physical descriptors, occupation, employer information, agency identifiers, ORI numbers, OCA numbers, transaction information, identification documents, fingerprints, and other information required by the requesting agency or applicable governmental authority.

Notary and Document Service Information: Identification information, documents presented for notarization, transaction information, journal entries, signatures, and other records required or permitted by applicable law.

Passport Photo and Identification Services Information: Photographs and information necessary to provide the requested identification or passport photo service.

Wedding Officiant and Ceremony Information: Names, contact information, ceremony details, marriage license information, photographs, videos, testimonials, and other information voluntarily provided in connection with wedding officiant services.

Online Form Information: Information submitted through our website forms, pre-registration forms, contact forms, appointment forms, questionnaires, service intake forms, consent forms, and other electronic forms.

Payment and Transaction Information: Billing information, payment status, transaction records, and related information. Payment card information may be processed directly by third-party payment processors. We do not intentionally store complete payment card numbers on our own systems.

• Maintain, administer, secure, troubleshoot, and improve our website and business operations;

Our website may allow individuals to submit personal information through online forms, including fingerprinting pre-registration forms, appointment requests, service intake forms, contact forms, questionnaires, and consent forms.

Information submitted through an online form may be transmitted to and processed by our website provider, hosting provider, form provider, scheduling provider, communications provider, cloud service provider, or other service providers that assist us in operating our business.

You are responsible for providing accurate and complete information.

Submitting an online form does not guarantee acceptance of an appointment, completion of a service, issuance of a license, approval of an application, successful processing of fingerprints, or any particular background screening result.

Please do not submit information that we have not requested or that is unnecessary for the service you are seeking.

5. FINGERPRINTS, BIOMETRIC INFORMATION, AND SENSITIVE PERSONAL INFORMATION

Certain fingerprinting and background screening services require us to collect and process fingerprints, identification information, demographic information, and other sensitive personal information.

We collect and process this information only as reasonably necessary to provide requested services, comply with applicable requirements, transmit information to authorized recipients, maintain required business records, and fulfill other lawful purposes.

Fingerprint images, biometric information, Social Security numbers, government-issued identification documents, background screening information, and other sensitive personal information are not used by Broward Fingerprinting for advertising or promotional purposes.

Fingerprinting and background screening information may be transmitted to governmental agencies, authorized vendors, requesting agencies, or other authorized recipients as necessary to provide the requested service.

We use reasonable administrative, technical, and physical safeguards appropriate to the nature of the information we handle. However, no method of electronic transmission or storage can be guaranteed to be completely secure.

6. PAYMENT INFORMATION

Payments may be processed through third-party payment processors and financial service providers.

Those providers may collect and process payment card information, bank information, billing information, and other transaction information according to their own privacy policies and terms.

Broward Fingerprinting does not intentionally store complete payment card numbers on its own systems.

We may retain transaction records, payment confirmations, invoices, receipts, refunds, and other financial records as necessary for accounting, tax, dispute resolution, fraud prevention, and legal compliance purposes.

7. THIRD-PARTY SERVICE PROVIDERS

We do not sell fingerprints, biometric information, Social Security numbers, government-issued identification information, background screening information, or other sensitive personal information for advertising purposes.

9. PHOTOS, VIDEO, AUDIO, TESTIMONIALS, AND MEDIA

In connection with certain services, events, ceremonies, mobile service visits, community activities, or other business-related activities, photographs, video recordings, audio recordings, testimonials, reviews, or other media may be voluntarily provided to us or captured with appropriate permission.

We may use identifiable photographs, videos, audio recordings, testimonials, names, likenesses, or other media for website, social media, advertising, marketing, promotional, educational, print, or digital purposes when the individual has provided appropriate consent or when otherwise permitted by applicable law.

Consent to promotional use of identifiable media is separate from consent to receive our services unless otherwise clearly disclosed and legally permitted.

We do not use fingerprint images, biometric information, identification documents, Social Security numbers, background screening information, or other confidential service records for promotional purposes.

Individuals who have consented to promotional use may contact us to request that we discontinue future use of identifiable media. We will make reasonable efforts to remove content from websites, social media accounts, and other media under our direct control.

Removal requests may not result in the recall or removal of materials already printed, distributed, published, shared, archived, or outside our reasonable control.

Additional terms governing authorized use of photographs, videos, audio recordings, testimonials, and other media may be contained in a separate Media Release or consent authorization.

10. SMS, EMAIL, AND SERVICE COMMUNICATIONS

If you provide us with a telephone number or email address, we may use that information to communicate with you regarding appointments, services, transactions, customer service matters, confirmations, reminders, and other service-related matters.

Where required by law, promotional or marketing communications will be sent only with appropriate consent.

You may request to stop receiving marketing communications by following the applicable unsubscribe instructions or contacting us.

Requests to stop marketing communications do not prevent us from sending communications necessary to provide requested services, complete transactions, respond to inquiries, or comply with legal obligations.

By submitting an online form, scheduling an appointment, purchasing a service, electronically accepting these Terms, or using our services, you acknowledge that you have been provided access to these Terms and agree to be bound by them.

Your acceptance of these Terms and acknowledgment of the Privacy Policy do not, by themselves, authorize Broward Fingerprinting, LLC to use your identifiable photograph, video, voice, name, likeness, or testimonial for commercial advertising or promotional purposes. Any such authorization will be obtained separately when applicable.
